Patient Centered Medical Home is designed to enhance the quality of care patients receive from their primary care providers. Hazelwood Family Medicine measures and reports this information to our patients and those who wish to become patients.

Keeping our patients up-to-date with cancer screening is important at Hazelwood Family Medicine. Our efforts have resulted in an increase in the number of patients receiving appropriate breast cancer and colon cancer screening. If you feel you may be out of date, please contact us so we can advise you on the appropriate screening activities.

High blood pressure is a largely asymptomatic disease that often quietly threatens your health. Be aware of the risk factors that could increase your chances of developing high blood pressure. The damage to your blood vessels from undetected or uncontrolled high blood pressure can lead to heart attack, stroke, heart failure and other serious health threats. Our goal is to help your control your blood pressure with an action plan that suits your needs that includes diet and exercise.

  • Friday - 8:00am - 3:00pm
